TEST REQUEST FORM

Please read the following statements and rules carefully.

I am registered with Student Disability Services (SDS) and I am approved for testing accomodations at Student Disability Services.

I understand that I must complete a "Test Request" form at least four (4) business days before each test date.

If I do not submit a "Test Request" form at least four (4) business days before the test date, then I will not be allowed to submit the "Test Request" form online.

To submit a late test request, I must go to the Student Disability Services office to complete a "Late Test Request Exception" form along with the "Test Request" form. I also must make an oppointment with the director of SDS to request approval to take the test in the SDS test center.

RULES WHILE TAKING TESTS

The rules for taking tests at SDS are the same as the rules in the classroom for all students.

  1. All tests will start at the same time as the class unless arrangements have been made to start early. If you are more than 15 minutes late for any test, SDS will need your professor’s written permission to administer the test after the class start time.

  2. Use the restroom BEFORE YOU START YOUR TEST.
  3. During tests only emergency restroom use will be permitted.
  4. NO cell phones in Test Center.
    A. If you bring a cell phone, then the cell phone must be put in your book bag or purse and put in a locker or on the bookshelf.

    B. Your cell phone must be OFF. Vibrate is NOT considered OFF. If your cell phone rings or vibrates while you are testing, we will interrpt your test and require you to turn off your cell phone.

    If you have a cell phone in your possession during a test, Student Disability Services will report you for cheating.


  5. NO hats in test rooms.

  6. NO textbooks, dictionaries, class notes, formula sheets, calculators, or other aids are permitted in test rooms unless approved by the professor for ALL STUDENTS taking the test.

  7. If a calculator is permitted, NO calculator cases/covers in test rooms. Calculator instruction cards/booklets must be inspected by SDS staff before entering test room.

  8. ONLY SDS scratch paper is permitted for tests.

  9. The student must bring the appropriate scantron form (unless supplied by the professor), if needed.

  10. The time limit for the test will be strictly enforced. Proofreading and editing must be done within the test time limit.

  11. Students will be monitored during tests.
