ELECTRONIC TEXTBOOK (E-Text) REQUEST FORM

PLEASE READ AND UNDERSTAND GUIDELINES BEFORE GOING FURTHER

Note: I understand that I have to bring a photocopy of the sales receipt for this textbook, along with the syllabus for the course.

  1. I am registered with Student Disability Services (SDS) and I am approved for receiving E-Text.

  2. I accept responsibility for providing Student Disability Services e-text requests at least eight weeks before the semester begins to allow sufficient time to obtain or produce e-texts that will enable me to meet reading assignments by due dates on the syllabus.

  3. I understand that the textbook I am requesting may not be available from the publisher in audio format. I understand Student Disability services will make every effort to get the textbook in e-text or audio format, if available.

  4. I also understand that if the textbook is not available in audio or e-text format, I have the option of permitting Student Disability Services to rip, scan and copy my textbook on to a CD and format it as e-text. In this case, I understand that I will need a screen reading software.

  5. Recordings for the Blind & Dyslexic (RFB&D) software will not read e-text produced by Student Disability Services or e-text sent by publishers.

  6. After you receive the E-Text, EXCHANGE OF THIS OR ANY OTHER FILE, IN A MODIFIED FORMAT OR COPY THEREOF, IS A VIOLATION OF THE U.S. COPYRIGHT ACT. SEPARATE AUTHORIZATION MUST BE OBTAINED FOR EVERY USER OF THIS FILE, ITS MODIFIED FORMAT OR COPY THEREOF, AND SEPARATE PROOF OF PURCHSE OF THE TEXT MUST BE PROVIDED.
