ELIGIBILITY FOR ACADEMIC ACCOMMODATIONS To be eligible for disability-related accommodations, students must have a documented disability as defined by the federal Rehabilitation Act of 1973, Section 504, and/or the Americans with Disabilities Act of 1990 (ADA). Accommodations are available to students whose disabilities include, but are not limited to, attention deficit hyperactivity disorder, deafness and hard-of-hearing, physical disability and/or health-related impairment, psychological and/or psychiatric disability, specific learning disability (such as dyslexia), speech impairment, visual impairment, or other disabilities which require administrative or academic accommodations. Students seeking academic accommodations are required to provide recent documentation from an appropriate health care provider or professional (please see Student Disability Services DISABILITY DOCUMENTATION REQUIREMENTS ). The cost of obtaining documentation is to be paid by the student. If documentation is incomplete, nonspecific, or otherwise inadequate to determine the disability and/or reasonable accommodations, Student Disability Services will require additional documentation. After admission to the university, students who have a disability that may require disability accommodations must contact Student Disability Services for an intake interview to be eligible to receive disability accommodations. The student will be required to provide disability documentation during the intake interview at Student Disability Services. All information is confidential and will be used only to assist the student. To schedule an intake interview,
