Online Forms
Test Request Form
This form allows students to request special aide on tests. You must submit a Test Request at least 1 week prior to the test date. If you are submitting this test request less than 1 week prior to the test date, please be advised that we will not accept online requests. You must come in-person and fill in a late-test request.
Professor Letter Request
Complete this form to request your professor letters. Pick up your letters and bring to your professors. Exception: Student Disability Services will e-mail professor letters for: Web/On-line courses and courses taught at Rosen or regional campuses.
Notetaker Application
Fill out this form to be considered for an SDS Notetaker position. VERY IMPORTANT: When you submit this application, you must attach a copy of your notes. We cannot accept applications without notes. You may use the photocopier in Student Disability Services to copy the notes.
E-Textbook Request

Request Documentation Review
After admission to the university, students who have a disability that may require academic accommodations must submit current disability documentation along with a completed "Documentation Review Form" to Student Disability Services. Please see Student Disability Services Disability Doc Requirement. If the documentation meet SDS criteria, the student will be contacted to setup an Intake Interview. If the documentation does not meet SDS criteria, the student will be contacted to submit more documentation. All information is confidential and will be used only to assist the student.
Intake Questionnaire
If the documentation meet SDS criteria, the student will be contacted to setup an Intake Interview. Feel free to complete it online and print it out to bring with you, or you may come into the office and fill it out there. Note: This is only for those students that have been accepted after submitting the request for documentation review.
